If this option is not successful, then the patient is started on isotretinoin (Claravis, Amnesteem, Absorica, Myorisan, Zenatane). This sounds more like eczema with the “sand” description. My son who’s 2 has suffered since birth with the sand feeling. I found chilled fresh cut cucumbers rubbed on the worse spots helps tremendously. Also unscented lotions such as baby aveeno anytime you notice the “sand” feeling.
